It is my responsibility to inform The Milky Hub of any changes and/or information that is relevant to my lactation service

I grant permission to receive lactation service by my chosen counselor at the The Milky Hub. I may be asked about mine/baby’s medical history. The fitting will include an examination of my breast/ nipples to provide flange fitting recommendation. I grant consent for these services.

Carisa Myers, NICU RN, IBCLC is a subcontractor of The Milky Hub and uses Practice Better or Spruce platforms for counseling. You will receive a link to register on her platform after you book a consult through The Milky Hub's scheduling platform.
Photos and Videos
I may be asked for photos prior to my appointment. I grant permission to provide my counselor at The Milky Hub photos and/or videos as needed for assessment purpose.
I consent The Milky Hub to use photos I provide on their website or social media for educational purpose, no personal information will be shared. You have the right to opt-out of this.
Virtual Visits
For a digital platform to be considered HIPAA-compliant a Business Associate Agreement (BAA) needs to be in place. For accessibility purpose, we conduct our virtual flange fittings services over Google Workspace or Practice Better/Spruce. Scheduling is done over Calendly and Stripe for payment processing Google Workspace, Practice Better, and Spruce are HIPPA compliant. Calendly and Stripe are not HIPPA compliant but we will do everything reasonable to ensure your privacy on these platforms.
Calendly and/or Stripe may use your personal information such as first name, last name, email address, telephone number, and billing info to send you a calendar invite, service receipt, or email communication. This information will only be used for scheduling/ appointment purposes. This is at the risk of the client.
After your flange fitting, we will email you your results via the email you provided. You have the right to opt-out of this.
Sharing of Your Information
You have the right to include a third party in your lactation service but privacy can not be guaranteed.
You have the right to have a copy of your lactation service sent to anyone of your choosing at any time. Please contact us at: info@themilkyhub.com.
I acknowledge and consent The Milky Hub to use my information for teaching purposes. No personal information will be shared. You have the right to opt-out of this.
