Non-Discrimination Policy

The Milky Hub (and its subcontractors) do not discriminate against employees, business associates, or clients on the basis of race, color, ethnicity, national origin, ancestry, creed, citizenship status, sex, gender identity, sexuality, sexual orientation, gender expression, religion, physical ability, age, family status, or marital status.

What Personal Information do we Collect?

Prior to any services, we will collect an intake form from you. That form may collect personal information such as name, home address, email address, phone number, and pictures to help us better consult you.

You have the right to:
  • Share information with your family, close friends, or others involved
  • Share information in a disaster relief situation
  • Include your information in a hospital directory

We may also have to share your personal information:
  • In case of a health or safety emergency (example: if you’re unconscious), we may go ahead and share your information if we believe it is in your best interest.
  • For insurance and/or billing purposes
  • In the event of a legal audit and/or to comply with legal regulations and laws
  • We are allowed or required to share your information in other ways – usually in ways that contribute to the public good, such as public health and research.
  • We have to meet many conditions in the law before we can share your information for these purposes.
  • In the case of fundraising: we may contact you for fundraising efforts, but you can tell us not to contact you again.

Your information will not be sold, or distributed without your consent except as stated in this Privacy Policy.

Our Responsibilities

We are required by law to maintain the privacy and security of your protected health information (PHI). We will let you know promptly if a breach occurs that may have compromised the privacy or security of your information. We must follow the duties and privacy practices described in this notice and give you a copy of it.
